If you have lost your Certificate of Competency or it has been stolen a replacement certificate may be issued to you. You will need to explain the circumstances of the loss and if it has been stolen then you must make a report to the Gardai. If your certificate has been lost through shipwreck or fire there is no fee for the replacement certificate.
You will need to complete an application for the replacement of a Certificate of Competency or Proficiency which has been lost/stolen or destroyed.
Important notice: Incomplete applications may be returned unprocessed, by post. Therefore in order to avoid any undue delay in the processing of your application, please ensure all information/documentation detailed on the application form is submitted.
